Abstract

¿¬±¸¸ñÀû: 1996³âºÎÅÍ 2016³â±îÁö Æò±ÕÀç¿øÀϼö¿Í °£È£»ç ¹èÄ¡¼öÁØÀÇ ÃßÀ̸¦ ÆľÇÇÏ°í Æò±ÕÀç¿øÀϼö¿Í °£È£»ç ¹èÄ¡¼öÁØ°úÀÇ °ü·Ã¼ºÀ» ºÐ¼®ÇÏ¿´´Ù.

¿¬±¸¹æ¹ý: 1996-2016³â 8°³ Á¶»ç¿¬µµ¿¡ ¼öÁýÇÑ È¯ÀÚÁ¶»ç ÀڷḦ ºÐ¼®ÇÏ¿´´Ù. ¿¬±¸Ç¥º»¿¡´Â 2,266°³ Á¾ÇÕº´¿ø°ú ÀÌµé º´¿ø¿¡¼­ 1°³¿ù°£ Åð¿øÇÑ È¯ÀÚ 2,408,669¸íÀÌ Æ÷ÇԵǾú´Ù. Æò±ÕÀç¿øÀϼö´Â ÇØ´ç ±â°ü Åð¿øȯÀÚÀÇ Àç¿øÀϼöÀÇ ÃÑÇÕÀ» Åð¿øȯÀÚ¼ö·Î ³ª´©¾î °è»êÇÏ¿´´Ù. °£È£»ç ¹èÄ¡¼öÁØÀº ÀÔ¿øȯÀÚ 100¸í´ç °£È£»ç¼ö·Î °è»êÇÏ¿´´Ù. ´ÙÁßȸ±ÍºÐ¼®À» À§ÇØ Æò±ÕÀç¿øÀϼö´Â ¹ØÀÌ 2ÀÎ ·Î±×·Î º¯È¯ÇÏ¿´´Ù.

¿¬±¸°á°ú: Æò±ÕÀç¿øÀϼö´Â 1996³â 13.3ÀÏ¿¡¼­ 2016³â 9.6ÀÏ·Î 27.8% °¨¼ÒÇÏ¿´°í, ¼öµµ±Ç-´ëÇüº´¿øÀÌ 15.7ÀÏ¿¡¼­ 7.4ÀÏ·Î °¨¼ÒÇÏ¿© º¯È­ÆøÀÌ °¡Àå ÄÇ´Ù. °£È£»ç ¹èÄ¡¼öÁØÀº 1996³â ÀÔ¿øȯÀÚ 100¸í´ç 32.7¸í¿¡¼­ 54.8¸íÀ¸·Î Áõ°¡ÇÏ¿´´Ù. ´ÙÁßȸ±ÍºÐ¼®°á°ú Æò±ÕÀç¿øÀϼö°¡ ª¾ÆÁú¼ö·Ï °£È£»ç ¹èÄ¡¼öÁØÀÌ Åë°èÀûÀ¸·Î À¯ÀÇÇÏ°Ô ³ô¾ÆÁ³´Ù. Æò±ÕÀç¿øÀϼö°¡ 50% °¨¼ÒÇÒ °æ¿ì Áö¹æ-´ëÇüº´¿øÀÇ °£È£»ç ¹èÄ¡¼öÁØÀº ȯÀÚ 100¸í´ç °£È£»ç 13.72¸í Áõ°¡ÇÏ¿´°í ¼öµµ±Ç-´ëÇüº´¿øÀº 12.18¸í Áõ°¡ÇÏ¿´´Ù.

°á·Ð: Áö³­ 20³â°£ Æò±ÕÀç¿øÀϼö °¨¼Ò¿Í ±×·Î ÀÎÇÑ °£È£»ç ¾÷¹«·® Áõ°¡·Î ÀÎÇØ º´¿øÀÌ °£È£»ç ¹èÄ¡¼öÁØÀ» ³ôÀÎ °ÍÀ¸·Î Çؼ®µÈ´Ù. ±×·¯³ª ÀÌ·¯ÇÑ ¹èÄ¡¼öÁØÀÇ Çâ»óÀÌ Áõ°¡ÇÑ ¾÷¹«·®À» ÃæÁ·½Ãų ¸¸Å­ ÀûÁ¤ÇÑ ¼öÁØÀ̾ú´ÂÁö¿¡ ´ëÇÑ ÆÇ´ÜÀÌ ¿ä±¸µÈ´Ù. ¾ÕÀ¸·Î Æò±ÕÀç¿øÀϼöÀÇ º¯È­¸¦ ¹Ý¿µÇÏ¿© ÀûÁ¤ °£È£»ç ¹èÄ¡¼öÁØÀ» °áÁ¤ÇØ¾ß ÇÒ °ÍÀÌ´Ù.

Purpose: To analyze the effects of average length of stay (ALOS) on RN staffing.

Methods: Public data of patient surveys collected 8 times between 1996 and 2016 were analyzed. The sample included 2,408,669 discharged patients from 2,266 general hospitals. The ALOS for each hospital was computed by dividing the sum of inpatient days by the number of discharges. RN staffing was defined as the number of RNs per 100 inpatients. ALOS was transformed into base-2 logarithmic values for regression analysis.

Results: ALOS decreased from 13.3 to 9.6 days. Large hospitals in the capital region had the greatest reduction, from 15.7 to 7.4 days. RN staffing increased from 32.7 to 54.8 RNs per 100 patients. ALOS had an inverse relationship with RN staffing. Controlling for other factors, a 50% reduction in ALOS was associated with increases in RN staffing by 12.18 and 13.72 RNs per 100 inpatients in large hospitals in the capital region and elsewhere, respectively.

Conclusion: Hospitals may have to increase staffing to respond to the increased workload resulting from the shortened ALOS. It remains uncertain whether such increases in staffing were sufficient for the increased workload. Changes in ALOS should be taken into account when determining appropriate staffing.
